VISA STICKER APPLICATION

A. FOR INDIVIDUAL APPLICANTS:

Requirements for the Issuance of a Visa Sticker for Individuals:

a. Working Visa
b. Valid original passport for more than 6 months
c. Medical certificate issued by the Embassy’s accredited clinics.
d. NBI clearance (for travel abroad) is authenticated by the Department of Foreign Affairs of the Philippines. (2 sets of photocopies, A4 size)
e. 2-pcs. 2×2 size pictures with white background. The photo must be clear, sharp, and in focus; it must also show both edges of the face clearly, and the photo must show a full front view of the face and shoulders.

     i. If the applicant stayed more than three (3) months in the Philippines
     ii. Children ages 11 and below are exempt from medical requirements.

B. FOR AGENCY APPLICATION

All agencies must file only working visas.

Working Visa

a. Visa notice (original, scanned, email, or photocopy)
b. Valid original passport for more than 6 months
c. A medical certificate issued by the Embassy’s accredited clinics only indicates the name of the agency, refer to Appendix (c).
d. NBI clearance (for travel abroad) is authenticated by the Department of Foreign Affairs of the Philippines. (2 sets of photocopies, A4 size).
e. 2-pcs. 2×2 size pictures with white background. The photo must be clear, sharp, and in focus; it must also show both edges of the face clearly, and the photos must show a full front view of the face and shoulders.

C. QATAR POLICE CLEARANCE REQUIREMENTS

Qatar Police Clearance

a. Letter of request for police clearance, addressed to the Ambassador of the State of Qatar in Manila.
b. The letter should state the following to secure police clearance:

  • Purpose of securing police clearance
  • Entry and exit dates in Qatar
  • Details of a stay or work in Qatar
  • Contact details of the former sponsors or employers
  • Country requiring police clearance
  • Address and contact details of the applicant
  • Signature of the applicant

C. NBI Sundry card authenticated by DFA.

  • 2 sets of photocopies of A4-size bond paper per page
  • No back-to-back photocopies

d. Photocopies of the passport and its pertinent pages in connection to the applicant’s stay and travel in Qatar, including stamps of entry and exit dates from Qatar.

  • Departure stamps from the Philippines
  • Arrival stamps in Qatar
  • Departure stamps from Qatar
  • Arrival stamps in the Philippines

e. For a lost passport, please submit an affidavit of loss and a Bureau of Immigration Certificate; both must be authenticated by the DFA.
f. Fees: US$31.00 CASH (filing fee to be sent to Doha)
g. Police Clearance Certificate Fee: PPH 1,750.00
h. 5-pcs. 2×2 pictures on a white background
i. Photocopy of Qatar I.D. (QID), if any, or Residency Sticker (IQAMA)

NOTE:

  • Please note that the process of police clearance takes 2 to 3 months.
  • W Express shall collect all Qatar fees upon submission of Qatar Police Clearance requirements.
